Dean's Certification Forms:
When applying to graduate school, it is customary for the college or university to require a dean's certification form or statement of good standing. These forms may be completed by the Dean of Students or the Assistant Dean. Please send request information and/or forms to:

Requests for transcripts are processed by the staff in the Registrar's Office. Information about requesting a transcript is available on their website.
