The following forms and resources are available on our webpage. Please note a number of forms are currently unavailable and are being updated.
EVALUATION FORMS
GENERAL FORMS
Social Event Registration - All social events must be registered with the college. Event registration is due to the office of Greek Organizations the Thursday before the event by 12:00pm. The form, which represents a contract, must be signed by the appropriate members. The form can not be filled out and sent electronically.
Weekly Inspection - A weekly inspection of the chapter house must be completed every week by the house manager. The weekly inspection form is due each Thursday by 1pm. This form can be completed online.
New Member Registration - This form must be filled out and Turned into the office of Greek Organizations with the one time registration fee.
The following forms related to housing are available on the web.
House Closing form - To be completed prior to each major college break period.
Out of House Form - This release must be signed by the chapters alumni/housing corporation before an individual is allowed to live outside the house.
