Student-Athlete Information

Dear Student Athlete or Parent/ Guardian:

In order for our athletes to compete on our intercollegiate athletic teams, they must complete several forms online and submit them using CNAV.  No athlete will be allowed to participate until these forms are completed and submitted, which must be done every year.  Student-athletes may login into their respective CNAV account and complete the following forms:

• Agreement to Participate
• Authorization to Render Medical Treatment and Release of Medical Information
• Emergency Contact Card
• Proof of Insurance Card

Parents have the option to login into their respective CNAV account and complete the Proof of Insurance Card.

We would also like to take this opportunity to explain our insurance programs to you. Every Gettysburg College student-athlete must have health insurance up to the $5,000.00 level per injury; either through their parents/guardian, or purchased through the college. The athlete's insurance is always the primary coverage. In the case of accident or injury, while involved in intercollegiate athletics, the college has a secondary insurance policy that picks up after $5,000.00 and will cover these cases up to $75,000. The NCAA insurance will then cover up to a limit of $20,000,000. Forms for the College's secondary coverage will be given to our athletes by the Certified Athletic Trainers, and must be completed and returned to Mary Ann Remaley, Box 409, Gettysburg College, Gettysburg, PA 17325, within 60 days of the incident. Documentation, in the form of an Explanation of Benefits from your primary insurance carrier must be provided. Injuries that exceed these limits will be covered by a larger policy that the college carries through NCAA.
