Injured Employee Instructions
If you are injured on the job, it’s important to report the incident within 12 hours to your supervisor or Human Resources (HR). All injuries, no matter how minor, must be reported to ensure a timely claim submission to our Workers' Compensation insurance carrier. Failing to notify promptly may delay or deny your claim.
Steps to Follow After an Injury:
- Notify your Supervisor immediately after the injury occurs.
- Complete the following forms and submit them to HR as soon as possible:
- Injury Report (Note: Social Security Number is not required) (PDF)
- Workers’ Compensation Information (PDF)
- Notice of Rights and Duties (PDF)
- Submit the completed forms to Human Resources within 12 hours via:

- In-person: Drop-off at HR office
Gettysburg College has a panel of approved healthcare providers for work-related injuries. You are required to see a provider from this panel for the first 90 days after your initial medical visit. After 90 days, you may continue with a provider from the panel or choose a different provider (subject to approval).
In case of an emergency, seek immediate treatment at the nearest emergency facility. Follow-up care can then be scheduled with a provider from our Workers' Compensation Panel.

Supervisor Information
When an employee reports a Workers' Compensation injury, please ensure the following:
- Ensure the employee completes the three required forms:
- Injury Report (PDF)
- Workers' Compensation Information (PDF)
- Notice of Rights and Duties (PDF)
- Provide the employee with a copy of the Workers' Compensation Panel Provider List.
- Review the details of the incident with the employee to confirm all necessary information is included, such as:
- The location of the injury
- Any witnesses
- Submit the completed forms to HR within 12 hours by:
